I testify that the information provided on this application is accurate and truthful to the best of my knowledge. I hereby authorize Dash Delivery Inc to obtain a background check and department of motor vehicles report if they so desire. I authorize Dash Delivery Inc to call any of my former employers listed on this application in order to obtain information about my past on the job performance. I understand that the final stage of the application process will involve a drug test which I will be required to pay $40 for if I fail. If the drug test is passed than Dash Delivery Inc will pay for the test regardless of whether or not I am ultimately hired. I also understand that employment at Dash Delivery Inc will involve periodic drug and motor vehicle tests as well as future background checks at various certification renewal periods. Failure of any of these will result in immediate termination. I understand that false information may be grounds for not hiring me or for immediate termination of employment at any future point if I am hired.
